HIPAA Audit Log

With HST Practice Management versions 7.0 and greater we have enhanced the "Patient Audit Tracking."  This enhancement will track field by field changes, updates and deletions.  The information that is logged: Access Date and Time, User Id, Patient ID/Visit #, Application Area. Field level changes will be reflected as such: Old Value and the New Value.

The Actions that are logged: Retrieve, Update, Delete, and Add.  ‘Add’ entries are mainly tracked in the Grid Areas of HST Practice Management.  For example an Add would be a tracked if a Supporting Physician was entered in the Appointment form such as an Anesthesiologist.

Please note the audit log doesn’t log new appointments that are scheduled, but will track any changes made after the initial scheduling of the patient.

Charge Entry, Payments and Adjustments are also not logged due to be a transactional based forms, which when a transaction is posted cannot be changed.

Additionally any area of the application that is not related to Protected Health Information or Patient PHI is not logged, such as Payers, Physicians, and Inventory.

To review the Patient Audit Logs, run the OP5025-Audit Log Report.

Note: This will only reflect patient account changes made from Scheduling through the Patient Account screen. Any transaction-based activity such as charges, payments, and adjustments, will not be logged.
