The Pre Op form is the “Pre Op Dashboard”.  The form is designed to ensure that the patient information and communication process has been verified prior to the day of the procedure.

 

Patient Information

• This information flows over from the appointment.
• The Pre Op Comments may be used to document what has been discussed with the patient to prepare for their arrival.

 

Check List

Allergies recorded here display on the SC1003 - Center Appointment Schedule report.

Check off appropriate pre-op tasks. The date for each tasks' completion can be recorded as well.
